Compensation Claims
To file a claim during our regular business hours Monday – Thursday 8:00 am to 4:30 pm or Friday 8:00 am to 4:00 pm, please contact Webber & Grinnell at (413) 586-0111 or (800) 416-0003. If you have any paperwork you would like us to have, please fax it to the Commercial Lines Dept. at (413) 517-0019.
If you or one of your employees is injured on the job that has resulted in five (5) calendar days of disability, you must file an Employer First Report of Injury form (Form 101). You are required to file this form with the Department of Industrial Accidents (DIA) and your insurer or to us within seven (7) calendar days, not including Sundays and legal holidays from the time you are notified of the injury from the employee or, as noted above, after the fifth day of disability. You should also give a copy to the injured employee. This form can be obtained from your carrier or can be downloaded from the Department of Industrial Accidents). Go to forms and Select Form 101. If the injury results in just medical bills, or fewer than five (5) days of disability, you would report it to your insurer or contact us immediately. A form 101 is not required.
